Carrier or stroller is a route decision
Older European metro systems are stairs, cobblestones and narrow pavements, and a stroller becomes something you carry rather than push. A carrier handles all of that and frees both hands, at the cost of your back. Check whether your specific stations have lifts before deciding — many networks publish exactly that map, and it changes the answer city by city.
The morning photo
Take a picture of each child at the start of every day. If they get separated from you in a crowd, you can show someone precisely what they're wearing rather than describing it. Pair it with a card in their pocket carrying your phone number with the country code — old technology, works without a battery, and works when the child is too upset to recite a number.
Snacks are crowd control
Museum queues, delayed trains and restaurants that don't serve until eight are the actual difficulty of travelling with children, and being hungry turns every one of them into a bad hour. Carry more than the day looks like it needs. A refillable bottle each removes the second half of the same problem.
One thing a day, and a change of clothes in the day bag
The itinerary that works with children has one anchor activity and a lot of slack around it. Whatever else happens, the day succeeded. And the spare clothes belong in the bag you're carrying, not back at the hotel — the moment you need them, the hotel is forty minutes away.
